Worried about hair transplant shedding or losing grafts?

Seeing hairs come out after a hair transplant can be surprising and worrying. You may notice a hair attached to a scab. Or perhaps some short transplanted hairs appear during washing. Occasionally, someone catches or knocks the recipient area and wonders whether they have pulled out a graft.

These are common concerns during hair transplant recoveryFortunately, seeing a transplanted hair fall out doesn’t usually mean that you have lost the follicle responsible for future growth. There is an important difference between normal hair transplant shedding and actually dislodging a newly implanted graft.

Can hair transplant grafts actually fall out?

Yes but the timing matters. During the first few days after surgery, newly implanted grafts have not yet become firmly anchored into the tissue. Rubbing or trauma during this early period can therefore dislodge a graft. The risk then reduces as healing takes place. 

One study has examined this question directly and involved 42 hair transplant patients. Researchers deliberately tested graft anchoring at different stages after surgery. During the first two days, pulling on a transplanted hair consistently resulted in graft loss. By day six, pulling the hair no longer dislodged the graft. At day nine grafts could no longer be manually dislodged during testing (Bernstein and Rassman, 2006).

This is why the early recovery stage needs particularly care.

When are hair transplant grafts secure?

Healing happens gradually rather than at one exact moment. I regard the first few days as the most vulnerable period, with graft security increasing throughout the first week. The Bernstein and Rassman study found that grafts could no longer be manually dislodged by day nine (Bernstein and Rassman, 2006).

For patients, it’s vital to protect the recipient area carefully during the first 7–10 days and follow the aftercare instructions provided carefully. Having a hair transplant is a big investment for most so I often say to my patients they should avoid any risks in the first two weeks.

This is one of the most common concerns after hair transplant surgery. Small crusts naturally form around transplanted hairs during healing. These hair transplant scabs soften and come away as patient’s wash the scalp as instructed. It is common to see a short hair embedded within one. They can look like an entire graft but usually it is not. The visible hair shaft can come away but the follicular tissue needed for future growth remains underneath the skin.

Timing still matters. Bernstein and Rassman found that deliberately pulling an adherent crust could dislodge grafts during the first five days (Bernstein and Rassman, 2006). This is why firmly attached scabs should not be picked prematurely and should be softened gradually with washing instructions provided.

Should I remove scabs after a hair transplant?

Scabs should generally soften through the washing routine recommended. They should then be removed gently at the appropriate stage. It is important to apply gentle pressure when washing and to gently remove softened crusts rather than forcefully pick them.

Scabs and crusting is a normal part of FUE hair transplant healing. However, unusually thick or persistent crusting might indicate delayed healing and may need assessment (Romera de Blas et al., 2026).

The hair transplant shedding phase is different from losing grafts. Many of the short transplanted hairs shed out during the first few weeks after surgery. Published literature commonly describes this occurring at around three to four weeks after transplantation (Garg and Garg, 2021).

This can be unsettling because the transplanted area may initially look fuller and then appear patchy. However, it is the hair shaft that is shed while the follicle remains below the skin. The follicle then progresses through its normal growth cycle before producing new hair.

Is shedding after a hair transplant normal?

Yes it is normal. Some patients can retain quite a few of the transplanted hairs but many others will shed most of them. Neither pattern reliably predicts the final result.

The transplanted area can temporarily look similar to how it appeared before surgery. New growth then develops gradually over the following months. This is why a hair transplant result should not be judged after too soon.

Can anything reduce shedding?

Minoxidil has been studied around hair transplant surgery and may reduce early postoperative shedding or encourage earlier regrowth. However it hasn’t been shown to improve graft survival itself (Singh, 1998).

Platelet Rich Plasma (PRP) may also support transplanted follicles. Studies suggest it may improve early growth and potentially graft survival, although research remains limited and PRP protocols vary between studies (Garg, 2016; Sindhusen et al., 2025).

Neither treatment prevents a graft from being physically dislodged. They should both be viewed as potential ways to support follicles.

Fresh bleeding can be a sign that a graft has been dislodged. If it occurred due to the area being scratched, rubbed or knocked during the first few days then it makes graft dislodgement more likely.

A dislodged graft means the transplanted follicular unit has been physically pulled out of its recipient site before it has become securely anchored. It may have lost contact with the developing blood supply and therefore will not grow from that site.

Bleeding alone does not prove that a graft has been lost. Graft loss is more likely if there has been clear trauma, fresh bleeding and an obvious piece of tissue or graft coming away. A short hair attached to a dry scab later in recovery, without bleeding or significant trauma is much less concerning.

What if I accidently knocked or touched my grafts?

Minor accidental contact happens frequently. Sometimes patients catch the recipient area during sleep, while changing clothes or simply forget and place their hand on the scalp. 

A light touch does not mean that the transplant has been ruined. The timing and force matter much more. A significant scrape during the first day or two deserves more attention than gentle contact later in recovery. We know that graft anchoring strengthens considerably by day 9 (Bernstein and Rassman, 2006).

Shock loss refers to temporary shedding of existing native hairs around the transplanted area rather than loss of the transplanted grafts. Surgery can temporarily disturb the growth cycle of nearby hairs causing them to shed several weeks later. Recent research has stated shock loss in recipient areas commonly appears around 2-8 weeks after surgery, and regrowth can begin from around 3 months (Romera de Blas et al., 2026).

Does shedding mean my hair transplant has failed?

Postoperative shedding is an expected part of recovery and cannot be used to judge whether a transplant has succeeded (Garg and Garg, 2021). The transplanted follicles need time to progress through their growth cycle. New hairs then appear gradually over the following months. Each patient’s recovery and results timeline varies so comparing your scalp with photographs of others online is rarely helpful.

How can I protect my grafts after surgery?

The most important step is following appropriate hair transplant aftercare during the early healing period.

Patients need to avoid unnecessary pressure, scratching and rubbing of the recipient area. Washing should then progress according to the instructions provided by my team. There are particular timeframes for getting back to normal activities like wearing a hat, exposure to strong sunlight, getting back to the gym etc.

Having a supportive team after your surgery is just as important as what happens on the day of the procedure.

When should I contact my surgical team?

As a hair transplant surgeon I would want to know about significant trauma during the first few days, persistent bleeding or an obvious wound. You should also seek advice for increasing redness, worsening pain, significant swelling, discharge, numerous pustules or areas of scalp that appear unusually dark or damaged.

Recognised FUE complications include infection, folliculitis, bleeding, delayed healing, shedding and more rarely tissue injury or graft loss (Garg and Garg, 2021; Romera de Blas et al., 2026).

Hair transplant shedding and losing a graft are not the same thing. Grafts are most vulnerable during the first few day but we know they become increasingly secure as healing progresses. Seeing hairs attached to scabs or shedding during the first few weeks usually does not mean the transplant has failed.

Follow your aftercare instructions and contact your surgical team if there has been significant trauma, fresh bleeding or anything that seems unusual.
